Minimally invasive surgery (MIS) is gradually becoming the standard of care for surgical treatment nonsmall cell lung cancer (NSCLC). Compared with multiple incision approach, including conventional multiportal video-assisted thoracoscopic (mVATS) and robotic-assisted (mRATS), uniportal (uVATS) offers a range potential benefits such as straight target view from sagittal plane, better exposure, ergonomic instrumentation, reduced postoperative pain, faster recovery, shorter length hospital stay, earlier administration adjuvant therapy when necessary. However, uVATS remains challenging procedure since all instrumentation shares same access along videothoracoscope. The learning curve demands profound knowledge specific technical details comfort specialized equipment to avoid intraoperative complications, essentially vascular accidents. This article provides step-by-step description right upper lobectomy, reviewing practical tips tricks about operating room set-up, strategy, management operative sequence applied fissureless technique. technique based on author's preference experience therefore should not be taken only valid option. Our purpose make accessible most surgeons in their daily practice.
